09:00am - 06:00pm Smart Start Pre-K Registration

Registration for the 2016-2017 Albemarle Smart Start Pre-Kindergarten will be held at the Albemarle Smart Start Partnership located at 1403 Parkview Drive, Elizabeth City, NC (Click here for Google map) on the following date:
- March 21, 2016 from 9AM to 12PM, and 2PM to 6PM
Please bring the following items to complete application process:
- Child's birth certificate for proof of age
- Child's social security card
- Child's shot record (must be up to date)
- Verification of Income (2015 w-2 forms or 2015 income tax documents or pay stubs)
Eligibility: Must reside in Camden County, Child must be 4 years old on or before August 31, 2016.

You must submit at least one document from each of the following categories:
- Proof of Residence
  - Electric Bill
  - Telephone Bill
  - Cable Bill
  - Mortgage Statement
  - Lease Agreement
- Proof of Identification
  - Valid NC Drivers License
  - Government Issued Identification Card
